Contact your business counter:

Whatever the reason for the closure of your business, you should contact your business counter, which will initiate the procedures for you with the CBE to remove your company registration and VAT number.

If you wish, you can cease your VAT number yourself by completing form 604 C via MyMinfin. Please note that before completing form 604 C, you must first have taken the necessary steps to stop your business number with the CBE via your business counter.

More info here.
